The Community Health Program Manager (CHPM) plays an integral role for VYCC’s Food and Farm program, fostering partnerships and supporting healthy communities throughout Vermont. The CHPM will oversee partnerships of the Health Care Share (HCS), a public health initiative that provides fresh produce and poultry to over 400 families navigating food insecurity and diet-related diseases. That VYCC Corps Members grow, harvest, and pack these shares, all while gaining personal and professional skills makes this initiative all the more compelling. This position is all about connecting organizations, systems, and people with the goal of continuing to deepen and broaden the impact of the Health Care Share.
The CHPM’s is primarily responsible for managing existing partnerships and the development of new ones. Additional responsibilities include enrolling HCS participants; coordinating share distribution; collecting relevant data; mentoring young people working on our Farm Crews; and securing funding for the HCS.
